The information you provide will assist me in matching your family with the right puppy. Your information will remain private and is needed if you want to visit Crystal Creek or have any of your questions answered including Adoption Fee.  I will respond with answers to most of your questions when I receive a completed Application.  Thanks.

What is your experience with the German Shepherd breed?

Do you have any interest in breeding your dog?

Are you interested in showing your dog in Conformation?

Are you willing to spay or neuter this dog?  Why or why not?

Please discuss all the activities you would like to experience with your new pup. Include competition as well as protection training and family activities.

Please list all your animals that are at your home.  Include gender of dogs.

Where will your new pup stay during the day? At night?  How long will your pup be alone?

Is there at least a 5 foot fenced in area for exercise?

CCS Health Tip!

When taking your dog into an area that is inhabited by fleas and ticks rinse them down with ACV and water! The little pests hate the acid environment!  Use ACV in your dogs' food.
